The clinical history is personal information. Although it is not necessary to for the patient to submit the request himself or herself, it is must be collected by the patient or by a duly accredited contact.

 

 

How to request it

• In person
  Please go to the Clinical History and Archive Unit on the ground floor of the Maternity Tower, from Monday to Friday, 9:00-13:00.

How to collect it

 

• If the clinical history belongs to the person collecting it
  You should bring your DNI, passport, driving license or any other official document with a photograph identifying you.
• If the clinical history does not belong to the person collecting
  You should bring authorization signed by the holder, as well as the holder's DNI (or a photocopy) and your own DNI to prove that you are the authorized person.
• If the clinical history belongs to a minor who is younger than 16 years old
  Only parents or legal guardians may pick it up, and they should bring the family record book and their DNI or a document showing proof of guardianship.
• If the clinical history belongs to a person with disabilities
  You should bring the document that shows the name of the person who has legal guardianship and both DNI (of the person who has legal guardianship and of the person who is in his or her guardianship).
• If the clinical history belongs to a deceased person
  The family or related persons may have access to it, except if the deceased has specifically stated otherwise while living. They should bring the family record book showing how they are related and the DNI.
